Getting the right brace can make a major impact on your patient’s life

Our patients can count on us for ongoing support at all stages of their journey, from the first meeting to fitting, to final fine-tuning and ongoing brace care and expertise.

 

It starts with your referral or prescription


 

When an orthotic device is a required medical treatment your patient will need a referral OR prescription to be seen by a certified orthotist.

 
 
 

Referral

Who can refer: physician or appropriately licensed healthcare prescriber. 

To further improve treatment planning consider providing your treatment objectives with your prescription. For example: “immobilize the ankle, prevent falls”, or “offload the medial compartment of the knee.”

Upon the receipt of your referral, your patient will be contacted by a Boundless employee to schedule an assessment at their earliest convenience.


Prescriptions

Who can prescribe: physicians

Physicians need to complete Section 2 and 4 of the Assistive Devices Application Form (ADP) for patients requiring custom-made orthoses. Original signed forms must be provided to the patient, and accompany the patient to their orthotics appointment.

Faxes or copies of the ADP form are not accepted by the Assistive Devices Program.

For patients requiring non-custom-made orthoses (or do not meet the requirements of ADP eligibility*) a standard prescription may be provided, and accompany the patient to their orthotics assessment.

What your patient can expect


Assessment and measure/cast

This appointment usually lasts 1 hour. For more complex custom braces such as spinal or long leg braces (KAFOs) more time is required.

The orthotist will review the referral/prescription, patient’s medical history, conduct a physical assessment of ranges of motion, strength and gait analyses (if applicable). Baseline measurements will be taken. These may include functional tests, self-report measures, range of motion or ulcer grading and description.

Following the assessment, a cast of the affected limb will be taken. This is an essential part of the process because it gives us the exact shape and alignment for making the brace. For non-custom braces measurements will be taken. 

Costs and funding (ADP, WSIB, ect.) will be discussed with your patient, and a quote will be provided prior to fabrication as requested. Boundless provides our patients with documentation required to apply for reimbursement through their private insurance.


Fitting, treatment provision & evaluation

This appointment usually lasts 1-2 hours and will be scheduled 2-4 weeks following the initial assessment. For more complex custom braces such as spinal or long leg braces (KAFOs) more time is required.

During the fitting appointment the Orthotist will ensure the orthosis is comfortable, functions properly and appropriate footwear has been procured by the patient for use with the orthosis. 

Patients will receive educational resources including Caring for their Orthosis, Gradual Wearing Schedule, Donning Instructions (paediatrics) and their Warranty. Treatment outcome measures will be taken with and without the orthotic device.


Follow-Up

Follow-up appointments will commonly be scheduled with patients 3-5 weeks following provision of the orthosis. By this appointment most patients will be wearing their orthoses on a regular basis.

The Orthotists will make further modifications to the orthoses based on patient feedback and clinical findings.


Ongoing Care

Our relationships with our patients often last years and even decades, as their braces experience the wear-and-tear of day-to-day life and their needs change. 

Patients can be referred back to their Orthotist or call to schedule an appointment at any time if; a) they are experiencing skin breakdown or discomfort; b) their orthosis no longer meets their needs; c) requires repair; d) or no longer fits (either too big or too small). 

Patients are eligible for new new custom-made braces through the Assistive Devices Program (ADP) due to a) change in medical condition b) physical growth / atrophy c) normal wear. Signed ADP forms should be provided by the physician for access to this funding program.

Repairs to braces outside of the warranty period are subject to fees.
